Contract and Agreement

I/We, the “Undersigned”, by signature below, hereby contract and agree that any fruit, seed, limbs, seedlings, leaves, and or trees, hereinafter “Product”, harvested or to be harvested, propagated, collected, rented, leased, marketed, and or sold for commercial use and or for profit, both now and any time in the future, would be reserved to Jack Slevkoff, his descendants, heirs, and or assigns forever, hereinafter “Provider”, unless or until Provider gives up that right or a portion of that right for specified reasons in writing.

In return, Provider agrees to pay a reasonable wholesale value or an agreed amount (to be determined later) in exchange for the harvested leaves, seed, seedlings, fruit, limbs, or trees produced from the Product or from the offspring of the Product received from Provider.

After filling out and signing, make a copy for yourself/yourselves and send the original with “blue-ink” signature to:

Upon approval and acceptance by Provider, Provider will release and send that certain described Product requested by the Undersigned to the location specified in writing by the Undersigned. Upon written request of an Undersigned, Provider will send a copy of the approved and accepted Contract and Agreement with Provider’s signature thereon.

This Contract is accepted and approved by Provider when the Product is sent to the Undersigned and becomes effective on date Product is sent
